This form is for nonregistered taxpayers reporting documentary stamp tax on unrecorded documents. Every person who executes or issues instruments described below and has less than five taxable transactions per month may report documentary stamp tax using this tax return. There is a 10% penalty fee per month, no greater than 50% for non compliance

My only problem was that there wasn't enough language in the sample to describe the grantee in a quit claim deed for a divorce. I would think that situation for a QC deed is pretty common.
