Use this form to revoke a previously issued Power of Attorney, [General, Special, Durable, Parental, etc] whether recorded or not. If a Power of Attorney was recorded or referenced any (transfer of real estate or any legal or equitable interest therein, excluding leases), then the Revocation Notice must be recorded. I would not have ordered this form had I realised how limited the fields are for details. There is no room for elaboration of terms. The language only allows one grantor and one grantee, and the gender and quantity default construction is a poor choice. Be basic, but leave room for more.
