The book and page reference of the original Lis Pendens that was filed (KRS 382.290 & 382.470)
Preparation Statement as per (KRS 382.335)
Return address for after recordation (KRS 382.240)
Notary statement for submitter (KRS 382.470).

The site is easy to navigate and exceptional services. Unfortunately, they could find no information on a tract of land that I own, and they canceled the search and refunded my payment.

The form is easy to use. However, the quit claim deed form seems to be for parcel of land, because the word "real property" is not in the form.
