Review: I wanna give more stars because the required information is there, but the character spacing is disjointed on the first page, rendering a gap-filled, awkward-looking document. Also, the opening parenthesis for the first field on the first page is on the wrong line and is backwards, which sets the wrong tone especially since it's the first thing you have to fill out.

Review: This suited my purposes just fine. Instructions were clear and easy to follow. But,I would like to have had the ability to delete the many extra spaces on the final document ... for readability purposes.

Review: You had the generic document that I was looking for Yay! The "example" page was helpful and reassuring. The auto input sections of my document looked ok until i printed it and then it appeared to be out of alignment which is why my rating is lowered to 4 stars it would be nice to have the ability to correct the title (created by me) when downloading PDFs for an e-filing
