After a Notice of Contract has been filed and posted, second or third tier subcontractors may preserve their lien rights by serving a completed and signed Notice of Subcontract form on the contractor by certified mail or personal service. If the Notice is not filed and served, that second or third tier subcontractor will be barred from filing a claim of lien against the owner's real property. N.C.G.S. 44A-23.
The Notice of Subcontract contains: (1) the name and address of the subcontractor; (2) a general description of the real property on which the labor was performed or the material was furnished (such as street address, tax map lot and block number, reference to recorded instrument, or any description that reasonably identifies the real property); (3) a general description of the subcontractor's contract, including the names of the parties; and (4) a general description of the labor and material performed and furnished. N.C.G.S. 44A-23(b)(3).
Filing the Notice of Subcontract requires the contractor to send notice to second and third tier subcontractors within five days of paying a subcontractor on the project. Id. The purpose of this notification is to provide notice that money is being exchanged between the parties.

The process was very easy but I did not realize before I paid to get this form , that I could have gotten it free from Tulsa county clerks office. Other than that, it was fine.

You're right that the Tulsa County Clerk publishes a free transfer on death deed form, and we'd never want anyone to feel they overpaid. Our version isn't that same blank, though: it adds an alternate beneficiary designation so the property still passes as you intend if your first choice doesn't survive you, it's built to handle one or two owners, and it comes with a plain-language guide and a completed example. Needed a Limited Power of Attorney form for a real estate transaction in another state. Proper form came up immediately and was fairly easy to complete. I think the sample completed form should have been more completely explained in layman's language instead of legalese (such as person granting permission instead of grantor or something like your name and address and the person who will be signing on your behalf) but since the form was one price no matter how many ways it was printed out, it was fine. I just filled it out several ways and had it notarized and sent it to my sister. Whichever combination is appropriate she and the lawyer will have. I found the site easy to navigate

The affidavit guidance was a great help and helped reduce the stress that usually comes with dealing with legalese. The Preliminary Change of Ownership that CA requires is quite complex since it covers a hoard of situations. I was left with a bit of uncertainty, but I definitely wouldn't want to try it without guidance.
