Use this form to discharge a previously recorded mortgage of record. (The certificate of discharge must contain a brief description of the mortgage and must state that the mortgage has been paid in full or otherwise satisfied and discharged and that the officer is authorized to discharge the mortgage of record.) (The mortgagee)/lender (shall present the certificate of discharge to the recorder for recording in the county in which the property is located within thirty days after the certificate of discharge is signed. The mortgagee may add the amount of the recording fee to the balance of the debt paid by the mortgagor.) This form can be (signed by the mortgagee, the mortgagee's executors, administrators, guardians, trustees, assigns, personal representatives, or special administrators appointed for that purpose, properly acknowledged or proved and certified as prescribed by chapter 47-19.) the deeds and related materials themselves are excellent but the PDF application is awful plus there is no way to customize the documents for specific purposes, so I had to type them from scratch in each instance.

Deeds.com did a great job in explaining exactly what I'd need to file a deed transfer (quitclaim deed). I didn't have to order the forms piecemeal, but was able to order the whole package at once for a reasonable price. These forms made it so easy to update the property deed and the instructions and sample filled out form were most helpful. You might want to add some brief information on when or why to use the Acknowledgment in Individual Capacity notary form. In my case the notary was required to use it but also filled in the brief notarize section on the Affidavit as well. She said the one on the Affidavit had some value because it showed she had witnessed the my signature. But this was only after I suggested both be filled in as she initially thought to just strike through it and just use the Acknowledgment in Individual Capacity form.
