Upon completion of the contract or improvement and upon receipt of the contract price, the contractor must deliver by registered mail or otherwise to the owner or owners of the real property a sworn affidavit stating that all liens have been paid in full or will be paid in full no later than ten (10) days from the date a bill is rendered for such services. Tenn. Prop. Code 66-11-205. The sworn affidavit must be executed and signed in the presence of a notary public.
The notice identifies the parties, the location and nature of the work or improvements, relevant dates, and anything else necessary for the specific situation.

I used the Oklahoma Gift Deed transferring property intra-family, and found it easy to complete. I could not find an Oklahoma Affidavit for the new law re citizenship verification, 60 O.S. Sec 121 and found it at another site that was not a fill in online. Oh well. Site was easy to navigate.

It was very easy to order,download, and print. The only issue I have is that the guide that came with my form really did not help me filling it out. I feel the explanations could have been better and suited more for the standard person. I was still confused when filling it out and will probably have to get a lawyer to make sure it's filled out correctly

I would absolutely use this service again. It was very convenient and I was pleasantly surprised at how responsive the staff was letting me know updates to my recording package. Also, my documents recorded immediately. I did have trouble uploading my document in the beginning because I didn't realize it had to be a pdf file. Thank you for the prompt instructions on the download and installation. The only problem I had was trying to input data into the form once I renamed the form.and saved it. I was unable to change the size of the font and was very frustrated. In the end I finally had to redo the entire form through Word using your format.
