When a Release of Lis Pendens document is filed it must contain a proof of service which is an acknowledgment of service by the person served or a certificate of service. In some cases, the clerk will permit a document to be filed but will require the proof of service to be filed promptly thereafter. 9.5(d)

I got a blank, a sample and detailed instructions, I'm happy. If the recorder's office had a form as they like to see, with your name as they like to see, and the property name as they like to see, no one would ever pay a lawyer for this but a little time to look up the exact names and this package you're all set. I recommend this because, while it isn't difficult, making a mistake could be very bad so getting the details right for a particular county is well worth the cost.

The form was short, and explainable.. so that is my feed back on that...but we have not received anything back to actually see if we filled the form out correctly. I wasn't sure what I was looking for initially so I printed out the wrong thing. Had to pay again to get the right one but much less expensive than getting it from a lawyer. Our military lawyers will make the forms official but they don't have the forms. Hope this makes things easier for our children when we pass. Thank you for offering this service.
