Every person who executes or issues instruments described below and has less than five taxable transactions per month may report documentary stamp tax using this tax return. There is a 10% penalty fee per month, no greater than 50% for non compliance

(Not the fault of Quit Claim Deed Forms however more scenario information or details regarding the forms on the site would be helpful.) Second I used this site to purchase this form specifically for Polk County expecting it to be partially completed with the Polk County information.
