What is a request for a sworn statement of account?
Under section 713.16 Fla. Stat. (2016), a statutory Request for a Sworn Statement of Account is used by property owners in response to a notice of intent to place a lien on the owner's property.
The request is addressed to the lienor and contains a description of the owner's property, the name of the contractor, and the name of the lienor's customer (if different from the owner). The form is then signed and dated by the owner. The demand to the lienor must be served on the lienor at the address (and to the attention) of any person designated to receive the demand as stated in the notice to owner. Failure or refusal to provide the statement as directed will not deprive the lienor of his or her lien. (713.16(2)).
A response to the form from the contractor must include a written statement, given under oath, of his or her account showing the nature of the labor or services performed and to be performed, if any, the materials furnished, the materials to be furnished, if known, the amount paid on account to date, the amount due, and the amount to become due, if known, as of the date of the statement for the improvement of the identified real property. (713.16(2)).
The lienor must furnish the requested statement within 30 days of receipt. If the lienor furnishes a false or misleading statement, the lienor will lose the right to recover attorney's fees in any action to enforce the claim of lien. See 713.16(5)(b).

After some initial general confusion -- (we sold a small piece of land privately and therefore do not typically prepare such documentation (!)) -- we were able to purchase and download all forms from Deeds.com and understand how to complete it/them. The help is all there, we just needed to read and study it - the "Example" helped alot. We were able to complete the Document per your online form(s) and then take it to be signed/notarized - and take the completed paper document to the Registry -- and it is now all registered and we are All Set. rn Took the morning (only). The form is easy to use. However, the quit claim deed form seems to be for parcel of land, because the word "real property" is not in the form.

The Deed of trust form was fine but the promissory note was less user friendly since I needed to change a few things that were fixed in the template. I ended up using white-out after I got no response when I emailed the help site that was provided in one of your emails, so it looks a little odd but should be usable

Well, we are 10 days from leaving the country for months and needed to notarize and record deed changes to our rental properties. We worried about USPS, UPS, DHL, etc. and hardcopies in the County's bureaucrats' hands. Soooo, we learned of Deeds.com from the County web site via one of the bulk digital recorders telling me about Deeds.com. Hit their site, read their instructions, concluded my tiny brain and decrepit abilities could handle the chore. WITHIN AN HOUR OF UPLOADING EVERYTHING INCLUDING C.C. FOR PMT IT WAS RECORDED AND I printed out copies. WORTH the $$ in speed, convenience and PEACE of mind. Pardon the loud trumpeting.

Working with one document at a time every thing was great, but the program will not let multiple documents save independently. When I saved a document and created another document the changes I made on the second document were on the 1st document. No big deal if your printing, but if your saving to email later, its an issue.
