Although a plaintiff (person initiation suit) or plaintiff's attorney usually files a Lis Pendens notice, Illinois states any party to the action may file a Lis Pendens (signed by any party to the action or his attorney of record or attorney in fact, on his or her behalf) Sec. 2-1901. Requirements of an Illinois Lis Pendens include; (the title of the action, the parties to it, the court where it was brought and a description of the real estate) Sec. 2-1901. A Lis Pendens is considered constructive notice once recorded in the county where the property is located, subsequent buyers or lien holders would be bound by the outcome of the case, (for the purposes of this Section, be deemed a subsequent purchaser and shall be bound by the proceedings to the same extent and in the same manner as if he or she were a party thereto) Sec.2-1901 Often cases include multiple plaintiff and defendants, this form has adequate space for multiple parties.

Communication is hard. The reps need to be empowered and encouraged to call the customers when necessary. They encourage 300 dpi resolution and under 2 MB PDF file, which is not even possible with our scanner. They made a vague comment about a legal description looking abbreviated but did not explain. They refused to call me. They said the county said "Image is light please darken", but the image looked fine to me. Maybe not their fault, but they refused to help work with the county on that for me. I followed their suggestion though and re-scanned at 300 dpi, but they misunderstood me and did not re-submit it right away. Over 48 hours later, it's still not recorded yet. I hope it will be today.

I have one suggestion and couple of question
I would think that most TOD's would be from married couples. It would be real helpful to have a example of the I(we) block for married couples.
Why would I check or not check the "property is registered (torrents)"

It would be helpful if the numbers on the instruction sheet were on the form. I was confused on page two if the signatures were for witnesses or buyer (grantee).
I do like the form and will use it in the future.
Also page one Grantee's signature only has one line and if there are two buyers need another line.
