Illinois Subcontractor's Final Waiver and Release of Mechanic's Lien
Under 770 ILCS 60, Illinois' Mechanic's Lien act, lien waivers are used between a contractor (including a sub or general) and a property owner to induce or encourage final or partial payments in exchange for forfeiting the claimant's right to a mechanic's lien.
Use the Final Lien Waiver after the confirmed final payment on an outstanding mechanic's lien. A subcontractor signs this form and provides to the owner and/or prime contractor in exchange for final payment on the job.

I called the county clerk office yesterday that i wanted to get a deed e-filed and recorded. You had the generic document that I was looking for Yay! The "example" page was helpful and reassuring. The auto input sections of my document looked ok until i printed it and then it appeared to be out of alignment which is why my rating is lowered to 4 stars it would be nice to have the ability to correct the title (created by me) when downloading PDFs for an e-filing

The pdf form is good; however, the input boxes merge into the line above so the text is hard to read when complete. I added a return before entering my data and this solved the problem.
