Illinois Unconditional Waiver and Release of Mechanic's Lien
Under 770 ILCS 60, Illinois' Mechanic's Lien act, lien waivers are a type of document used between a contractor (including a sub or general) and a property owner to induce or encourage final or partial payments in exchange for forfeiting the claimant's right to a mechanic's lien.
Contractors may use this form to waive and release a right to a mechanic's lien. A contractor waives and releases its lien and right to claim a lien for labor, services, or materials on a construction project described in the document.

You form was good the only thing that did not work was the download to fill it out !!! I use a Mac system and maybe that's what the issue was. I filled it out by hand and it worked for the purpose .

Great to have a downloadable form, rather than a cloud solution that gives no guarantee of privacy. Appreciated the sample.......but all of that still left me with open issues about how to tweak the form to serve my particular needs......for example: how to ensure that survivor rights were properly characterized; how far back I should go with the "Source" section + how I should layer my own additions to the chain of ownership, etc. Nonetheless, an overall happy experience. Very easy process for both quit claim and beneficiary deed forms and explanations.. also gives an example form filled out to ensure you have something to go by when filling out your own for accuracy.. the forms are for your specific county, city, and state so no problems when you go to record at deeds office!
