(5) Five Page General Durable Power of Attorney, following Indiana Code and subjects listed below.
By this General Durable Power of Attorney, you name an attorney-in-fact with power to act on your behalf pursuant to Indiana Code 30-5, as it exists and is amended in the future.
(a) REAL PROPERTY. Authority with respect to real property transactions pursuant to Indiana Code 30-5-5-2
(b) TANGIBLE PERSONAL PROPERTY. Authority with respect to tangible personal property transactions pursuant to Indiana Code 30-5-5-3
(c) BOND, SHARE AND COMMODITY. Authority with respect to bond, share. and commodity transactions pursuant to Indiana Code 30-5-5-4,
(d) BANKING. Authority with respect to banking transactions pursuant to Indiana Code 30-5-5-5, including but not limited to the authority:
(e) BUSINESS. Authority with respect to business operating transactions pursuant to Indiana Code 30-5-5-6, including the power:
(f) INSURANCE. Authority with respect to insurance transactions pursuant to
Indiana Code 30-5-5-7.
(g) BENEFICIARY. Authority with respect to beneficiary transactions pursuant to
Indiana Code 30-5-5-8.
(h) GIFTS. In the event I become permanently mentally incapacitated, to make gifts of my property and to have general authority with respect to gift transactions as provided in IC 30-5-5- 9
(i) FIDUCIARY. Authority with respect to fiduciary transactions pursuant to Indiana Code 30-5-5-10
(j) CLAIMS AND LITIGATION. Authority with respect to claims and litigation pursuant to
Indiana Code 30-5-5-11.
(k) FAMILY MAINTENANCE. Authority with respect to family maintenance pursuant to
Indiana Code 30-5-5-12.
(l) MILITARY SERVICE. Authority with respect to benefits from military service pursuant to
Indiana Code 30-5-5-13.
(m) RECORDS, REPORTS AND STATEMENTS. Authority with respect to records, reports and statements pursuant to Indiana Code 30-5-5-14, including, but not limited to, the power to execute on my behalf any
(n) ESTATE TRANSACTIONS. Authority with respect to estate transaction pursuant to
Indiana Code 30-5-5-15.
(o) DELEGATING AUTHORITY. Authority with respect in delegating authority in writing to one (1) or more persons as to any or all powers given to the attorney-in-fact by this General Durable Power of Attorney document, pursuant to Indiana Code 30-5-5-18.
(p) TAX MATTERS. Authority: (a) to prepare, execute and file on your behalf.
(q) SOCIAL SECURITY ADMINISTRATION. Authority to act as your representative and attorney-in-fact for all matters involving the Social Security Administration and benefits from the administration.
(r) ALL OTHER MATTERS. Authority with respect to all other possible matters and affairs affecting property owned by you pursuant to Indiana Code 30-5-5-19.

easy to use, no problems except in beneficiary box. Need to make the box bigger because I have 4 beneficiaries to list. how do I enlarge the box.

I had a really good experience. I bought two forms from a different website and they didn’t help me at all. Granted, I only paid the minimum here I bought the upgrade and they showed me how to fill it out and what it should look like I went and had it notarized and took it to the county recorder and everything went smoothly. Highly suggest.

The Personal Representatives Deed is definitely a helpful document for my files. I find it need just a little tweaking by deeds.com , There should be more space for the legal description. I did see in the FAQ's you recommend putting it in the Exhibit and this is what I did. Also I couldn't get the year to be accepted and had to write it in. These are just some minor suggestions, on the whole I was grateful to find this document. Thank you.

I purchased the Notice of Completion form because the City of Chula Vista did not have a "fillable" version of this form on their website. The Deeds.com version of this form is somewhat different than the City's version (8 numbered paragraphs vs. 11 numbered paragraphs.) However, it contains the same information in a different format. This form provided more blank space to fill in important items- like a long ownership name- than the version on the City's website. The recorder's office was satisfied with this form as I hand-delivered it to the Recorder's Office and they approved it for recording. Overall, I found this form easy to use and found the extra blank space for writing on the form helpful. My one comment for possible improvement is: it would be even more helpful (particularly for attorney users) to have strike-out capability. I would have liked having the ability to strike-out inapplicable portions of long awkward sentences. Still, I would use this form again.

It was frustrating to get the scans done but that might have been due to a learning curve. After 4 tries they were accepted. I couldn't figure out how to delete or close the failed attempts. Waiting to see if Recorder office has changed the title.

The form and instructions were easy enough to follow if you had all the information.
The only drawback to the form was the length of text allowed for the name of the document (#4). The form self populates in multiple locations but when printed truncated the name if too many characters were used. I kept having to update the name of the document to allow for proper printing.
