The name of the party(s) releasing the Lis Pendens (KRS 382.200)
The name of the party(s) being released (KRS 382.470 & 382.200)
The book and page reference of the original Lis Pendens that was filed (KRS 382.290 & 382.470)
Preparation Statement as per (KRS 382.335)
Return address for after recordation (KRS 382.240)
Notary statement for submitter (KRS 382.470).

After some initial general confusion -- (we sold a small piece of land privately and therefore do not typically prepare such documentation (!)) -- we were able to purchase and download all forms from Deeds.com and understand how to complete it/them. The help is all there, we just needed to read and study it - the "Example" helped alot. We were able to complete the Document per your online form(s) and then take it to be signed/notarized - and take the completed paper document to the Registry -- and it is now all registered and we are All Set. rn Took the morning (only). Great service ! Hawaii is not a "forms state" so unfortunately the public has no way to get templates on our local gov site but deeds.com to the rescue. The template was affordable and easy to use and successfully recorded. Great to use when you don't need to involve title or attorneys for simple deed changes, thank you
