The name of the party(s) being released (KRS 382.470 & 382.200)
The book and page reference of the original Lis Pendens that was filed (KRS 382.290 & 382.470)
Preparation Statement as per (KRS 382.335)
Return address for after recordation (KRS 382.240)
Notary statement for submitter (KRS 382.470).

After some initial general confusion -- (we sold a small piece of land privately and therefore do not typically prepare such documentation (!)) -- we were able to purchase and download all forms from Deeds.com and understand how to complete it/them. The help is all there, we just needed to read and study it - the "Example" helped alot. We were able to complete the Document per your online form(s) and then take it to be signed/notarized - and take the completed paper document to the Registry -- and it is now all registered and we are All Set. rn Took the morning (only). I'm typing along and suddenly I can't fit anything more into the page and there's plenty of room. This is my 2nd time using this site. No problem the first time years ago. Now it's an issue, looks like I'll need a typewriter to finish the form. Where do I find a typewriter?!! I can't complete the legal description!

I tried to use the free stuff you find on the internet. You quickly find out that free is rarely ever (never) free. Even worse, the long term cost can be immeasurable. Glad I realized that before I got too far in. Do yourself a favor, spend a few bucks up front to get the right forms.
