Preparation Statement as per (KRS 382.335)
Return address for after recordation (KRS 382.240)
Notary statement for submitter (KRS 382.470).

Great service ! Hawaii is not a "forms state" so unfortunately the public has no way to get templates on our local gov site but deeds.com to the rescue. The paperwork was fine but it was just paperwork. Georgia does not accept a paper form. The form must be transmitted electronically through a registered account. Using an attorney may have been the simple way to file the cancellation.

Downloaded and used the Ladybird Warranty Deed for a county in Florida with no issues. Cost for the download and subsequent recording fee of the deed totaled less than $40. No reason to pay hundreds. I assume the subsequent transfer upon death will go smoothly, but I of course, will never know. The "example" of a completed form was very beneficial. Also, get a copy of the current deed and make sure legal description of real estate is exactly the same on the new deed.

All I needed was a simple Certificate of Trust. Deeds.com had a template for exactly what I needed. I didn't have to make an appt with an attorney, wait for one to be available, nor pay a ridiculous amount for a standardized document. Super easy.
