Return address for after recordation (KRS 382.240)
Notary statement for submitter (KRS 382.470).

The process was not difficult but I don't think that it suited my needs. There were several fields that were not applicable to me but I had to enter something to proceed. I also filled out the other form and mailed it in with some documentation that the electronic service did not ask for. Questions of my attempt are still unanswered. I hope I didn't waste time with this process. We shall see. Thank you.

Not good. The blanks on the form, supposedly especially tailored for the specific county, didn't have enough space for a document name or the doc #. I had to retype the whole doc myself.

We are also reviewing the field spacing for the prior document title and recording number so we can improve the form. No further action is needed from you.

I wanted to do an Enhancement Life Quitclaim deed and the county government lady didn’t know what I was even talking about. Great and fast service. Would have been grate to have seen a little more detail or a pre-filled sample in the fields.
