The Notice of Furnishing is used to certify that labor and/or materials were furnished, or will be furnished upon a specific property identified in the Notice.
In Kentucky, any lien will be dissolved unless the claimant, within six (6) months after he ceases to labor or furnish materials, files, in the office of the county clerk of the county in which the building or improvement is situated, a statement of the amount due him, with all just credits and set-offs known to him, together with a description of the property intended to be covered by the lien sufficiently accurate to identify it, the name of the owner, if known, and whether the materials were furnished or the labor performed by contract with the owner or with a contractor or subcontractor. K.R.S. 376.080(1).

Very good information. Easy access and easy to download. All the forms needed for TOD to be notarized and recorded with the county office. Much better than working with a Trust and the expense of lawyers, especially when several parties are involved and the owner of said property knows exactly to whom the property should go. Having forms and instructions available for the public to have their wishes recorded and confirmed makes handling final planning much easier and prevents family members from having the unnecessary task of going through court to solve property distribution issues. I'm typing along and suddenly I can't fit anything more into the page and there's plenty of room. This is my 2nd time using this site. No problem the first time years ago. Now it's an issue, looks like I'll need a typewriter to finish the form. Where do I find a typewriter?!! I can't complete the legal description!

The Affidavit- Death of Joint Tenant form you provided is not the same form as showed on the Los Angeles County property tax website. It appears that the LA county form requires entering additional info that is not included in your form.
