Affidavit of Identity and Survivorship for Transfer on Death Deeds -- Minnesota Form 50.2.3
Minnesota's transfer on death deeds are governed by Minn. Stat. 507.071.
To complete the transfer of property rights after the grantor owner's death, the beneficiary must file an affidavit of identity and survivorship in the county where the land is located (subd.20). The affidavit must contain details about the deceased grantor owner, all beneficiaries, surviving or deceased, a legal description of the real estate, recording information from the transfer on death deed, and the address of the individual or entity who should receive future tax statements.
In addition to the affidavit, include an official copy of the grantor owner's death certificate and a clearance certificate from the Minnesota Department of Human Services. All three forms are required, and may be filed as a unified document with references on the affidavit to the death certificate and clearance certificate, but each may also be recorded individually. Note that the transfer is not completed until all three forms are filed for record.
The beneficiary may complete an online request from the DHS website: https://edocs.dhs.state.mn.us/lfserver/Public/DHS-5893-ENG to obtain the clearance certificate.
Individual counties may require additional information -- check with the local agency responsible for maintaining land records with any questions.

Deed.com is an amazing site. After calling many places and going on many websites to figure out what I needed to submit (most counties cannot help with questions and the place I needed to turn the documents into could not help either, they are not allowed to give legal advice) I came across Deeds.com. From Pennsylvania here. Documents are great and easy to fill out however you are lacking a couple of things. You only provide the option for a Grant Deed when you purchase by your county which is Mercer County for me. Why not give the ability to get a Warranty Deed that better protects the Grantee? Also, being from Pennsylvania and in a county that mined Buituminous Coal we are required to include the Coal Severance Notice and Bituminous Mine Subsidence and Land Conservation Act Notice. You can check the box on your Deed form that they are required and attached but you do not provide the verbiage or form for this. You state that you know what each county requires and include everything required but you do not include these two required Notices. This has been a requirement for years and the wording never changes. I had to look for these Notices and hand type this information and include it on another seperate page after the Notary section on the Deed. The Grantor has to sign the Coal Severance Notice and be witnessed by a Notary so I had to add another place for the Notary and will have to pay twice for witnessed signatures when it could have been included in your document. My Deed from 2003 was done that way and then the Notary statement after that so it was only one notarized witness of signature.

I thought the forms were reasonably priced, the instructions included in the packet were thorough, and the examples helpful. Thank you for the additional CDR forms too. I contacted the Recorder's office via email with a question and Jennifer Bowser answered promptly. Job well done! However, when I delivered the deed and Real Property Transfer Declaration to the Clerk's office in Lafayette, the clerk was unfamiliar with the Declaration document being submitted and it took some time to convince her to submit the form without charging the recording fee. She even tried to phone the recorder's office for clarification, but no one answered. There then was an additional form at that office that I had to complete called Recording Request/Transmittal Form. I would suggest including that form with instructions in your on-line packet to speed up the process when a Deed is delivered to the County Clerk's satellite office. I do not expect every clerk to know all the particulars of recording requirements but a little knowledge wouldn't hurt.

These products would be more useful if they final deed could be copied and pasted into a word document for proper formatting. Because most of the document is protected against selecting and copying, I did not find it useful. I would not purchase again.
