Affidavit of Identity and Survivorship for Transfer on Death Deeds -- Minnesota Form 50.2.3
Minnesota's transfer on death deeds are governed by Minn. Stat. 507.071.
To complete the transfer of property rights after the grantor owner's death, the beneficiary must file an affidavit of identity and survivorship in the county where the land is located (subd.20). The affidavit must contain details about the deceased grantor owner, all beneficiaries, surviving or deceased, a legal description of the real estate, recording information from the transfer on death deed, and the address of the individual or entity who should receive future tax statements.
In addition to the affidavit, include an official copy of the grantor owner's death certificate and a clearance certificate from the Minnesota Department of Human Services. All three forms are required, and may be filed as a unified document with references on the affidavit to the death certificate and clearance certificate, but each may also be recorded individually. Note that the transfer is not completed until all three forms are filed for record.
The beneficiary may complete an online request from the DHS website: https://edocs.dhs.state.mn.us/lfserver/Public/DHS-5893-ENG to obtain the clearance certificate.
Individual counties may require additional information -- check with the local agency responsible for maintaining land records with any questions.

Really good product, included guide to filling out forms. Totally pleased with that part. Customer service however was terrible. Did not hear back after I sent two emails. The site signed me up but after I was accepted they would not allow me to download a form, with the notation my account was closed. Had to use another email. Had problems with that. Finally got off of site and went to a login site that allowed me to download the forms. If you can get past setting up your account, it is fantastic site. Nice price compared to alternatives. Also I recieved two validation codes. Have no idea why they were sent.

I found there were a large number of documents available to download. The file naming on the PDFs could be more descriptive, and it would be nice to be able to download a complete set with one click.

This is another great deal that has come out of the quarantine for covid. Saved me hours and days of time. and provides a way to file deeds that really isn't done effectively anyway else. It was also very inexpensive that you would not expect. I didn't even have to leave home.

I found this site a must. It provided all the forms I needed to file a Quit Claim Deed. I filed what use to be called a Quick Claim Deed 30 years ago. You only had to file the one form. Today it is called a Quit Claim Deed. The pack provided forms that I had no idea had to be filed with the Quit Claim Deed. I would not have known this otherwise if the option hadn't presented itself. Thank you for the prompt instructions on the download and installation. The only problem I had was trying to input data into the form once I renamed the form.and saved it. I was unable to change the size of the font and was very frustrated. In the end I finally had to redo the entire form through Word using your format.
