A death certificate names a person and a date. It says nothing about a parcel, nothing about a recorded deed, and nothing about who owns the real estate now. Neb. Rev. Stat. Section 76-2,126 closes that gap: when Nebraska real estate passes under a previously recorded transfer on death deed, a cover sheet is attached to the death certificate and recorded with it, carrying the identifying facts the certificate itself cannot supply. This package prepares that cover sheet.
Four items the statute names
Section 76-2,126 is short and specific. The cover sheet indicates the title of the document, the previously recorded document data, the grantor, the surviving grantee, and the legal description of the property being transferred. The form follows that list in the same order: the deceased transferor and the date of death, the surviving grantee or grantees with mailing addresses, the recording date, instrument number, book and page, and county of the recorded transfer on death deed, and the full legal description of the parcel. A street address blank rides along because the same address is reported to the county assessor on the transfer statement that accompanies the filing. Nebraska prescribes no official statutory cover sheet, so the content of the statute is the only specification there is, and several county registers of deeds publish sheets built on the same items.
The filing that closes a transfer on death deed
Ownership does not wait on this paperwork. Under Section 76-3415 the property passes to the designated beneficiary at the transferor's death, by operation of a deed already sitting in the record. What the recorded death certificate and cover sheet accomplish is public: the register of deeds indexes the death against the property and against the names in the chain of title, and a later buyer, lender, or title examiner reads a complete story instead of a deed that appears never to have taken effect. The transfer on death deed being documented, sometimes searched as a TOD deed or a beneficiary deed, was signed before two disinterested witnesses and an officer and recorded during the transferor's life; it is a separate recorded instrument and is not included in this package.
What travels with it to the counter
The cover sheet does not go alone. Section 76-214 attaches a Real Estate Transfer Statement, Form 521, to be filed at the time a certified or authenticated copy of the grantor's death certificate is filed, and the register of deeds does not record where that statement is missing. Documentary stamp tax is computed and collected at the counter under Section 76-903, and the exemptions listed in Section 76-902 include transfer on death deeds and certified death certificates, with a claimed exemption shown on the face of the document or by documentary proof and certified by a signed statement. Recording fees run ten dollars for the first page and six dollars for each additional page under Section 33-109.
Nebraska formatting, before the counter sees it
Section 23-1510 keeps the top three inches of the first page clear for the register of deeds recording information and places the title of the instrument and the return address below that band; where either is missing, Section 23-1503.01 lets the register add a page and charge the additional page fee. Section 23-1503.01 also governs paper and ink: black ink on white paper of at least twenty pound weight, pages between 8.5 by 11 and 8.5 by 14 inches, and legibility sufficient to produce a readable copy. This form is letter size, reserves the full three inch band, holds one inch side margins, and sets every glyph at twelve points or larger.
A recorded document with no signature line
Section 76-2,126 states what the cover sheet indicates and stops. No signature, witness, or acknowledgment formality for the cover sheet appears in that section, and the form carries neither a signature block nor an acknowledgment certificate; the attached certified death certificate is the certified record of the fact reported. The signature in this package's filing belongs to the transfer statement, which Section 76-214 has signed by the grantee or an authorized representative under a declaration made under penalties of law.

The fill-in pdf produced a messy document and several of the fields too small to accommodate the text. A tailorable MS Word document would have been far better. I would not recommend using this product.
