The Affidavit- Death of Joint Tenant form you provided is not the same form as showed on the Los Angeles County property tax website. It appears that the LA county form requires entering additional info that is not included in your form.

I was very pleased with the document I downloaded. I was able to edit it easily and save a copy both as a permanent copy or one that could still be edited. Great and fast service. Would have been grate to have seen a little more detail or a pre-filled sample in the fields.
