Requesting a Lien Waiver in Oklahoma
Sometimes it's necessary for a property owner to request a lien waiver from a contractor in exchange for paying the contractor any amount owed. With a waiver, contractors agree to give up their rights to file a lien against the title for any outstanding balance due.
A property owner may request that a contractor provide the owner with a lien waiver from each and every person who performed labor or furnished materials for the improvement of the property. The request form identifies the parties, the project's location, and relevant dates. Deliver the request via a US Mail service with delivery confirmation.
Failure to provide lien waivers from these parties may allow the property owner to withhold payment to the contractor in the amount of any unpaid claims for labor or materials.

Was warned by attorney that forms from internet have lots of mistakes. But after looking all over, took a chance on here. So far, I am satisfied, and actually happy that I got something that (I believe) meets my state and local requirements. Haven't filed the deed yet, or had to put it into effect, but being able to pick the local area, and have the relevant state law listed on the deed, gives me confidence. Also, got the whole package of possibly relevant forms, and a very good guide how to prep the deed with a sample completed deed - greatly appreciated!

I had overwhelming emotions taking my deceased wife's name off my condo, so it took me a year to steel myself to submit the form. You think you're purchasing 1 form for $25 but you are getting several which explains the $25. My printer ran out of black ink and I couldn't change the color of the text so that it would print. I couldn't copy and paste it to Word and work on it there. I'm going to purchase ink today so that it will at least print right. I will have to retype the text in Word; not good.
