The request form identifies the parties, the project's location, and relevant dates. Deliver the request via a US Mail service with delivery confirmation.
Failure to provide lien waivers from these parties may allow the property owner to withhold payment to the contractor in the amount of any unpaid claims for labor or materials.

Fortunately, the link continued to work, so after I discovered this, I downloaded the corrupted files again, and they now seem fine. I do not know if my computer or the website caused this odd problem.
