Upon completion of the contract or improvement and upon receipt of the contract price, the contractor must deliver by registered mail or otherwise to the owner or owners of the real property a sworn affidavit stating that all liens have been paid in full or will be paid in full no later than ten (10) days from the date a bill is rendered for such services. Tenn. Prop. Code 66-11-205. The sworn affidavit must be executed and signed in the presence of a notary public.
The notice identifies the parties, the location and nature of the work or improvements, relevant dates, and anything else necessary for the specific situation.

This is not at all the form that I needed. I am trying to disclaim my interest in a property, but this form is much too rigid to work for my case. It would have been nice to know some of the more specific details before purchasing the document.

I have one suggestion and couple of question I would think that most TOD's would be from married couples. It would be real helpful to have a example of the I(we) block for married couples. Why would I check or not check the "property is registered (torrents)" Do I need a notarized signature of the Grantee

Very good information. Easy access and easy to download. All the forms needed for TOD to be notarized and recorded with the county office. Much better than working with a Trust and the expense of lawyers, especially when several parties are involved and the owner of said property knows exactly to whom the property should go. Having forms and instructions available for the public to have their wishes recorded and confirmed makes handling final planning much easier and prevents family members from having the unnecessary task of going through court to solve property distribution issues. Creating my enhanced lifde estate deed was rather easy but I am not happy. After buying the 3 form package for about $29 and crating my legal form... was not happy with your disclaimer than you cannot guarantee the form will meet required legal standards. I was convinced you all knew what you're doing since the forms are supposedly specific to Polk county, Florida.
