Upon completion of the contract or improvement and upon receipt of the contract price, the contractor must deliver by registered mail or otherwise to the owner or owners of the real property a sworn affidavit stating that all liens have been paid in full or will be paid in full no later than ten (10) days from the date a bill is rendered for such services. Tenn. Prop. Code 66-11-205. The sworn affidavit must be executed and signed in the presence of a notary public.

Great forms - I got several property deeds and really appreciated that they came with the required state forms (for NY). the sample completed form is also really helpful. Attorney wanted hundreds- with this form it is the same amount of work but I can file myself for the cost of lunch!!

We have been waiting for a Title Company to put a release of Lien together for the past 3 months. I figured it was taking way to long and decided to use template here instead. In less than hour I was able to add all the information on the template and provide forms to our Seller to use. We were buying and he didnt think they were necessary. But I refused to pay him in full until he agreed to sign papers at the bank, and of course in front of a notary. We turned around and filed the Release of lien paperwork at County Clerks office, we officially own our house. Thank you!
