Transferring Title with the Utah Affidavit of Survivorship
Utah's affidavit of survivorship is governed by Section 57-1-5.1 of the Utah code. When lawfully executed and recorded, this document passes title of property held as joint tenants to the survivor(s) after the death of one of them.
Under 57-1-5.1, the affidavit must contain specific information: it must cite the interest being terminated; contain a legal description of the property; refer to the recording information of the deed creating the interest; include an official copy of the deceased owner's death certificate; and be recorded in the recorder's office for the county where the property is located.
This document is important because it helps to maintain a clear chain of title (ownership history) for the property, which should, in turn, help to simplify future transactions. Each circumstance is unique, so contact an attorney with specific questions or for complex situations.

instead of the rep giving me instructions on how to summit the documents,with 3 pages, he or she told me i had all night to figure it out!!! wow...because of that i rate the service very poorly...fast to get it done but very poor customer service...so...i give them a 2.5 rating.

Really solid system for determining what may prevent your documents from being accepted. I love the comments section allowing for fluid communication. I only wish there were automated emails for all those communications and once documents were accepted, but I did receive a couple personally-generated emails regarding the progress instructing me to check the site.

Wonderful forms. It's nice that they were formatted perfectly for my county, it's real easy to miss a requirement (margines, font size, and so on) and end up with a rejection or higher recording fee. Good job folks!

The Quit Claim Deed for the state of Ohio worked for me, saving me the cost of an attorney doing it. O.K., maybe that wouldn't have amounted to more than a few hundred dollars, but anywhere I thought I could save money (and learn something new on top of it) is something I want to do.
That said, be forwarned. While I'm not an attorney I'm not averse to spending many hours researching the lingo found in this kind of form and thoroughly understanding exactly how everything has to be filled in.
I should add that my ex-wife and I remain friends and she was the one giving me the property/house (thus, technically I filled out the forms on her behalf). Because there was no personal conflict, it made it easier to undertake.
Lastly, what others have said about the county office where you must file a Quit Claim Deed not being helpful, that's true in the sense that they do not want to be instructing non-attorneys on filling out the necessary forms. I did take a preliminary draft set of the forms to the county office but was VERY CAREFUL about explaining that I only needed a couple of questions answered about procedure for submitting the final documents. They were helpful once I made it clear I wasn't asking them for "legal advice". And their help was critical as the final submittals requires stopping at three different offices (MapDocuments, Auditor and finally the Recorder's office).
So I say thank you to Deeds.com. Their service for the Quit Claim Deed was invaluable.
