This form is used by the Beneficiary/Lender to appoint a substitute Trustee for a deed of trust.
(The beneficiary may appoint a successor trustee at any time by filing an appointment of trustee or a substitution of trustee for record in the office of the county recorder of each county in which the trust property or a part of the trust property is located.) (Utah Code 57-1-22 (1)(a))
57-1-22. (2) An appointment of trustee or a substitution of trustee shall:
(a) identify the trust deed by stating:
(i) the names of the original parties to the trust deed;
(ii) the date of recordation; and
(iii)(A) the book and page where the trust deed is recorded; or (B) the entry number;
(b) include the legal description of the trust property;
(c) state the name and address of the new trustee; and
(d) be executed and acknowledged by all of the beneficiaries under the trust deed or their successors in interest.

Good site. Two things to note. 1. The Documentary Transfer Tax Exemption sheet, the word "computer" is used when I think it should be "computed" Error in state form? 2. The California Trust Guide could have a watermark which is less distracting. Kind of hard to read the print with the DEEDS.COM logo so prominent.

Your forms are great but I couldn’t use them with my computer and acrobats reader didn’t work. And then my thermal printer would only print 1/3 size pages with no way to fix it. I ordered a new color printer and we’ll see what happens.

That shrunken page is typical of thermal printers: most use narrow receipt or label paper, so a full page gets scaled down to fit, and no setting can change that. Thermal paper also fades over time, which you don't want on a document you're recording, so the new printer is the right call. When it arrives, print on plain white paper with “Actual size” selected in the print settings so the page comes out full size with its margins intact. If Acrobat Reader gives you trouble again, save the form to your computer first and open the saved file in Acrobat Reader rather than in your web browser. Still stuck? I downloaded the Lady Bird deed. The process was quick and easy to download. Just select your county, fill out the form. You will need the property description from your original deed. In my case I had to go downtown Wayne County (Detroit). (Make an appt online). 1st you will have to get the property tax certified to ensure all taxes are paid to date (5th floor at the Wayne County Treasurer office). Give them the form you just filled out and they will stamp certified $5. After that take the form to the Register of Deeds (7th floor) appt needed. $18. Make sure it is properly notarized and all signatures completed. Once approved, they will scan it, stamp it, give it back with a receipt and mail a copy also. All Done. Worked beautifully. My co worker go a lawyer and paid over $250. I just used deeds.com and total for forms and going downtown with notarizing was less than $40 Yea!
