The form was short, and explainable.. so that is my feed back on that...but we have not received anything back to actually see if we filled the form out correctly. the deeds and related materials themselves are excellent but the PDF application is awful plus there is no way to customize the documents for specific purposes, so I had to type them from scratch in each instance.
