Questions answered? Let's get started!

In Illinois, a contractor furnishing labor, services, material, fixtures, apparatus or machinery must provide a work notice to a residential owner setting forth the contract price, description of work, names and addresses of all subcontractors or material suppliers and other laborers and amounts due to each.

Under Illinois law governing mechanic's liens, the contractor has a duty to give the owner a statement in writing, under oath or verified by affidavit, of the names and addresses of all parties furnishing labor, services, material, fixtures, apparatus or machinery, forms or form work and of the amounts due or to become due to each. 770 Ill. Comp. Stat. 60/5(a). The property owner also has a duty to request this statement from the contractor before the owner (or his agent, architect, or superintendent) makes a payment to the contractor. Id. Merchants and dealers in materials are not required to make this statement. Id.

In regard to any owner-occupied single-family residence, 770 Ill. Comp. Stat. 60/5(b)(i) states that each contractor must provide the owner or his or her agent, (either as part of the contract or as a separate printed statement) with the following statement before the owner makes his or her first payment:

THE LAW REQUIRES THAT THE CONTRACTOR SHALL SUBMIT A SWORN STATEMENT OF PERSONS FURNISHING LABOR, SERVICES, MATERIAL, FIXTURES, APPARATUS OR MACHINERY, FORMS OR FORM WORK BEFORE ANY PAYMENTS ARE REQUIRED TO BE MADE TO THE CONTRACTOR.

If the owners of the property are persons living together, the statement is conclusively presumed given to each such owners if given to one of them. Id.

The Notice contains the following information:
1) Contractor's name and address;
2) Owner's name and address;
3) Description of work or materials provided by the contractor;
4) The start date of the project;
5) Address of the project;
6) The contract price; and
7) The names and addresses, description of services or materials provided, and amounts owed to each individual laborer, subcontractor, or other party with a potential claim.

The document must be verified by completing the included affidavit and sworn before a notary public. A certificate of service must also be completed to attest to the name of the recipient as well as the date and method of service (such as personal service or certified/registered U.S. mail).

To call an affidavit of minor correction a Correction Deed in your descriptions is incorrect. They are two different products. I did not intend to purchase an affidavit. I intended to purchase a Correction Deed.

1) I was very pleased when the staff mentioned your service since the three referenced on the Recorder's website all wanted HUGE Account set-up and maintenance fees AND BIG fees per recording, and yours has no set-up fee AND nominal per-recording fee; 2) My (few) recordings will be NON-LAND Related, summary or entire record(s) of Administrative (Procedures Act) records, Other than the Border width and Cover Sheet, do you anticipate any other special requirements for such recording(s)? NOTE: I just sent one by Snail Mail, and they just informed me that due to the GERMIPHOBIA 'Pandemic' the ONLY open and record Snail Mail ONCE A MONTH On the first of each chmonth!

A refreshingly easy service to use. They offer auxiliary forms as a courtesy. Theres no "gotcha" capitalism. You pay the reasonable fee and the needed forms are accessible instantly to download. I've used it twice so far and it worked perfectly!

Deed.com is an amazing site. After calling many places and going on many websites to figure out what I needed to submit (most counties cannot help with questions and the place I needed to turn the documents into could not help either, they are not allowed to give legal advice) I came across Deeds.com. It has been so helpful and I was able to research what documents I needed. I purchased one document and after more research I realized I needed a different document. Deed.com refunded my first purchase. I then purchased an Affidavit of Death and a Deed for the county and state I needed them for. Both the example and guide were very helpful and I will be submitting my documents after I have them notarized. I give five stars

I thought the forms were reasonably priced, the instructions included in the packet were thorough, and the examples helpful. Thank you for the additional CDR forms too. I contacted the Recorder's office via email with a question and Jennifer Bowser answered promptly. Job well done! However, when I delivered the deed and Real Property Transfer Declaration to the Clerk's office in Lafayette, the clerk was unfamiliar with the Declaration document being submitted and it took some time to convince her to submit the form without charging the recording fee. She even tried to phone the recorder's office for clarification, but no one answered. There then was an additional form at that office that I had to complete called Recording Request/Transmittal Form. I would suggest including that form with instructions in your on-line packet to speed up the process when a Deed is delivered to the County Clerk's satellite office. I do not expect every clerk to know all the particulars of recording requirements but a little knowledge wouldn't hurt.
