In Iowa, in order to perfect a mechanic's lien, the claimant must make a filing with the clerk of the district court of the county in which the building, land, or improvement to be charged with the lien is situated of a verified statement of account of the demand due the person, after allowing all credits. I.C. 572.8(1).
The verified statement must include: 1) the date when such material was first furnished or labor first performed, and the date on which the last of the material was furnished or the last of the labor was performed; 2) the legal description of the property to be charged with the lien; and 3) the name and last known mailing address of the owner of the property. Id. Once the lien is filed, the clerk of court will mail a copy of the lien to the owner. 572.8(2).

I tried 3 local attorneys and got no where , wrong information, to busy and another one was very rude. One said he'd do it then didn't. I was so stressed and tried a different online form company advertising an in person attorney within hours. They did call back but gave me the wrong answer. I needed a form used in NC and knew about it from the clerk of the court. The deadline was approaching, I looked one more time and found Deeds.com. Was warned by attorney that forms from internet have lots of mistakes. But after looking all over, took a chance on here. So far, I am satisfied, and actually happy that I got something that (I believe) meets my state and local requirements. Haven't filed the deed yet, or had to put it into effect, but being able to pick the local area, and have the relevant state law listed on the deed, gives me confidence. Also, got the whole package of possibly relevant forms, and a very good guide how to prep the deed with a sample completed deed - greatly appreciated!
