Durable Power of Attorney under the laws of the Commonwealth of Kentucky
1. Powers Regarding My Assets. My agent shall have the following powers with respect to my assets: (Subjects Addressed)
(a) Invest, sell, purchase, lease, borrow and encumber assets.
(b) Deal with real property.
(c) Collect and recover assets.
(d) Deal with insurance.
(e) Deal with financial institutions.
(f) Represent me in all tax matters.
(g) Employ others.
(h) Enter, establish, close, or maintain safe deposit boxes.
(i) Litigate.
(j) Miscellaneous powers.

A. Incidental Powers.
B. Ratification.
C. Third Party Reliance.
D. Revocation and Amendment.
E. Protective Proceedings.
F. Interpretation and Governing Law. This instrument is to be construed and interpreted as a General Durable Power of Attorney. The enumeration of specific powers herein is not intended to, nor does it, limit or restrict the general powers herein granted to my Attorney-in-Fact. For a third party to construe otherwise would be contrary to my intent. This instrument is executed and delivered in the Commonwealth of Kentucky and the laws of the Commonwealth of Kentucky shall govern all questions as to the validity of this power and the construction of its provisions. Nevertheless, I intend that this instrument be given full force and effect in any state or country in which I may find myself or in which I may own property, whether real or personal. I direct that my Attorney-in-Fact not be required to give bond and, if any bond is required, that no sureties be required.

Thank you for your service. It seems to have worked, I printed a document purporting to be the Deed I needed. I was somewhat disappointed though - I was expecting something as impressive as the Title Search, which goes back to 1828 and includes Millard Fillmore, admittedly not one of our most celebrated Presidents. The form was very easy to use and the sample tool you had was very helpful. the only problem i had was saving the document and then trying to find it later. I finally was able to figure out how to save it. but i still cannot find the saved document on my computer. Luckily i printed it before i closed it and did not need to make any changes.

I didn't have any problem downloading and filling out the form on my computer and printing it yesterday. I didn't know what to put for "Source of Title". I called the county recording office; they didn't know either and said to leave it blank. I got the form notarized at my bank and took it in to the recording office. They checked it, accepted it, I paid a fee, and it's done. So easy. My children will appreciate that I've done this. Added note: You do have one typo on your form--you left out 'at'. It should read: "You should carefully read all information at the end of this form."

The NV Clark County deed upon death was perfect! Our county doesn't offer a template, but rather has a long list of rules and specifications where they expect you to make your own document. I didnt want to risk making an unacceptable form so I purchased the template from Deeds.com. It was easy to use and very thorough. Our deed upon death was notarized and filed with the county with no issue. Save yourselves the time and headache and get the template!
