The Notice of Furnishing is used to certify that labor and/or materials were furnished, or will be furnished upon a specific property identified in the Notice.
In Kentucky, any lien will be dissolved unless the claimant, within six (6) months after he ceases to labor or furnish materials, files, in the office of the county clerk of the county in which the building or improvement is situated, a statement of the amount due him, with all just credits and set-offs known to him, together with a description of the property intended to be covered by the lien sufficiently accurate to identify it, the name of the owner, if known, and whether the materials were furnished or the labor performed by contract with the owner or with a contractor or subcontractor. K.R.S. 376.080(1).

I called the county clerk office yesterday that i wanted to get a deed e-filed and recorded. I was told they are not accepting documents in person because of the COVID 19 pandemic that I have to mail it. I found Deeds.com online and in less than 24hrs i have my document. Nice site, easy to maneuver around, and walks you thru completing forms. However, at the end, the forms listed poorly explained. It would be helpful to have better explanations of the forms so people know which forms they really need to download.

Not very helpful since I am married and the example provided is for single person. Nevada homestead requires spouse to sign off on quit claim deed but no guidance provided as to where this acknowledgment is placed on template form. There should be example for married person as well. Had to use another service. Waste of $21.

I find your forms hard to use, inasmuch as the forms cannot be converted to a Word Document. Editing and deleting of extra lines is not possible, making for a deed with large blank spaces. Document that results is not usable for me.
