Under Nebraska Revised Statutes section 76-214, deeds and other instruments transferring title submitted for recording must be accompanied by a statement containing details about the transfer. Form 521, found on the websites for most counties or at http://www.revenue.nebraska.gov/PAD/forms/521_Real_Estate_Transfer_Statement.pdf meets most of these requirements.
Transfers resulting from a property owner's death, such as those under transfer on death deeds or joint tenancies, may require additional information (76-214(1), (2)). In those situations, including a cover sheet that contains the extra details assists the recording office and reduces the chance of delays in processing.

It was easy to download the form I wanted BUT there were 2 other options listed for "open/download." I didn't want to risk more charges for something I couldn't determine I needed so I passed them up. There were a few others listed with the option to "view" so I did that, without down-loading, and there were no additional charges. I would've liked that opportunity for 2 others that didn't offer "view" so maybe deeds.com missed a sale?

I found this site when looking for help filing a quitclaim deed to change my property deed to my married name. I received the correct forms, an example filled out, and a guide specific to my state. I am not so happy. I did find and purchase the document I needed. But there is one problem. It is in Adobe PDF format only. I cannot enter information into the form.
Sorry to hear that. Sounds like you may have been trying to complete the document in your browser instead of downloading the PDF and completing it on your computer. The PDF forms are fill in the blank, that's one of the reasons we use that format.
