Use this form to create a survivorship joint tenancy in Ohio real property. The Ohio Revised Code Section 5302.17 states that a "deed conveying any interest in real property to two or more persons . . . creates a survivorship tenancy in the grantees, and upon the death of any of the grantees, vests the interest of the decedent in the survivor, survivors, or the survivor's or survivors' separate heirs and assigns."
Once executed and recorded, this deed immediately conveys ownership from the grantor to the grantees. To retain a shared ownership interest in the property, the grantor simply conveys the property to herself and the other intended co-tenants.

Very good information. Easy access and easy to download. All the forms needed for TOD to be notarized and recorded with the county office. Much better than working with a Trust and the expense of lawyers, especially when several parties are involved and the owner of said property knows exactly to whom the property should go. Having forms and instructions available for the public to have their wishes recorded and confirmed makes handling final planning much easier and prevents family members from having the unnecessary task of going through court to solve property distribution issues. I purchased a Quit Claim Deed package a couple weeks ago. The included guide unfortunately didn't answer all the questions about my specific case of how to fill it out, so I sent them a couple questions on Sept 8. It's now the 23rd, and still no reply. The form is a useless waste of money if I don't know how to fill it out in a legally-accurate way.
We certainly do not want you to waste your money Philippe, to that end your order and payment has been canceled. We do hope that you seek the advice of a legal professional familiar with your specific situation.
