Use this form to create a survivorship joint tenancy in Ohio real property. The Ohio Revised Code Section 5302.17 states that a "deed conveying any interest in real property to two or more persons . . . creates a survivorship tenancy in the grantees, and upon the death of any of the grantees, vests the interest of the decedent in the survivor, survivors, or the survivor's or survivors' separate heirs and assigns."
Once executed and recorded, this deed immediately conveys ownership from the grantor to the grantees. To retain a shared ownership interest in the property, the grantor simply conveys the property to herself and the other intended co-tenants.

This was such a money saver. I was told by someone at the courthouse that I had to have a lawyer prepare the paper work for my mom. They stated that family members couldn't prepare the papers. I was hopeful when I found that I could prepare the survivorship affidavit on Deeds. I was able to prepare everything myself and had no issues today when at the courthouse for all the changes. You think you're purchasing 1 form for $25 but you are getting several which explains the $25. My printer ran out of black ink and I couldn't change the color of the text so that it would print. I couldn't copy and paste it to Word and work on it there. I'm going to purchase ink today so that it will at least print right. I will have to retype the text in Word; not good.
