Illinois Subcontractor's Final Waiver and Release of Mechanic's Lien
Under 770 ILCS 60, Illinois' Mechanic's Lien act, lien waivers are used between a contractor (including a sub or general) and a property owner to induce or encourage final or partial payments in exchange for forfeiting the claimant's right to a mechanic's lien.
Use the Final Lien Waiver after the confirmed final payment on an outstanding mechanic's lien. A subcontractor signs this form and provides to the owner and/or prime contractor in exchange for final payment on the job.

I had overwhelming emotions taking my deceased wife's name off my condo, so it took me a year to steel myself to submit the form. My rating is not a 5. Although it had good instructions, it would NOT print the whole document no matter how many times I inputted the names. I ended up writing it in to complete. I also recommend putting it on one page. I had to pay an additional fees per page and if I had to notarize it, why did I have to find 2 witnesses as well. I deserve a discount for the time I spent repeatedly putting the same data. I was trying to save money since Im on social security only. It didnt. Get it to work correctly

I recommend you add a box "Add another document or package." The way it is now confused me, so I uploaded the same document two more times, thinking the upload failed the first two times.
