The Notice of Furnishing is used to certify that labor and/or materials were furnished, or will be furnished upon a specific property identified in the Notice.
In Kentucky, any lien will be dissolved unless the claimant, within six (6) months after he ceases to labor or furnish materials, files, in the office of the county clerk of the county in which the building or improvement is situated, a statement of the amount due him, with all just credits and set-offs known to him, together with a description of the property intended to be covered by the lien sufficiently accurate to identify it, the name of the owner, if known, and whether the materials were furnished or the labor performed by contract with the owner or with a contractor or subcontractor. K.R.S. 376.080(1).

Easy to use but the quit claim deep looked old and dated. The example of how to fill out should have asterisks stating what is need and what can be skipped

Sorry that this a little late. I'm VERY HAPPY with everything. The deeds paperwork was just what I was looking for. It was very to fill out, it was different than n the folks used years ago. Form and instructions were useful. But I suggest creating a form for transferring a deed pursuant to a trust. The existing form is based on a will going through probate so it doesn't fit the trust situation in some respects.

At first glance, explanations and guidance to fill out the grant deed seems quite direct and no too difficult. I did not see any reference to a mortgagee which I believe needs to be incorporated in a boundary line adjustment (BLA), though not sure I'll do the actual filling out the form in the next couple of weeks and will be in a better position for a more complete review.
