After a Notice of Contract has been filed and posted, second or third tier subcontractors may preserve their lien rights by serving a completed and signed Notice of Subcontract form on the contractor by certified mail or personal service. If the Notice is not filed and served, that second or third tier subcontractor will be barred from filing a claim of lien against the owner's real property. N.C.G.S. 44A-23.
The Notice of Subcontract contains: (1) the name and address of the subcontractor; (2) a general description of the real property on which the labor was performed or the material was furnished (such as street address, tax map lot and block number, reference to recorded instrument, or any description that reasonably identifies the real property); (3) a general description of the subcontractor's contract, including the names of the parties; and (4) a general description of the labor and material performed and furnished. N.C.G.S. 44A-23(b)(3).
Filing the Notice of Subcontract requires the contractor to send notice to second and third tier subcontractors within five days of paying a subcontractor on the project. Id. The purpose of this notification is to provide notice that money is being exchanged between the parties.

Very easy to use and no hidden costs. You get to download whatever you need and can save it and even reuse it. So it's like having your own library of form that you pay for once. They even give you more related forms than you ask for and it turned out we needed some if those forms as well. The forms meet what our county requires for margins in records and so on. Price seemed high (~$28) for just some forms (especially because we may not actually use the forms), but it beats navigating the Hawaii state and Honolulu county websites for forms. It would be better if a single button push would download all 7 or 8 forms.

The Beneficiary Deed is easy to fill out, expecially with the examples/explanations provided. The only recommendation I would make is to state that the Parcel ID and the Assessor's ID are one in the same. I looked everywhere for something that mentions "Assessor's ID" in my paperwork to no avail. Upon calling the Maricopa Assessor's number in Maricopa I was told that they are the same.

The Mineral Deed transfer form was pretty good. Could have used more info in the guide about where to find legal property descriptions and source of title. Also more space on the pdf for entering return addresses - there was room for only one; I needed three. I will be sending the form to the County Courthouse soon. I hope it works.
