Upon completion of the contract or improvement and upon receipt of the contract price, the contractor must deliver by registered mail or otherwise to the owner or owners of the real property a sworn affidavit stating that all liens have been paid in full or will be paid in full no later than ten (10) days from the date a bill is rendered for such services. Tenn. Prop. Code 66-11-205. The sworn affidavit must be executed and signed in the presence of a notary public.
The notice identifies the parties, the location and nature of the work or improvements, relevant dates, and anything else necessary for the specific situation.

LOVE THIS!! I am a REALTORand from time to time I have had to take documents for filing. I'm so glad I invested some time online researching eFiling services. You charge too much for a form. Your business model is shortsighted. I would not try to use your service again. You got $20 from me this once, but I would try very hard to not use your service again. Your model does not encourage serial or professional usage.

It was frustrating to get the scans done but that might have been due to a learning curve. After 4 tries they were accepted. I couldn't figure out how to delete or close the failed attempts. Waiting to see if Recorder office has changed the title.
