Upon completion of the contract or improvement and upon receipt of the contract price, the contractor must deliver by registered mail or otherwise to the owner or owners of the real property a sworn affidavit stating that all liens have been paid in full or will be paid in full no later than ten (10) days from the date a bill is rendered for such services. Tenn. Prop. Code 66-11-205. The sworn affidavit must be executed and signed in the presence of a notary public.
The notice identifies the parties, the location and nature of the work or improvements, relevant dates, and anything else necessary for the specific situation.

This was the first time I had ever had to fill out a Warranty Deed, so if it was not for your example form on how to fill one out, I would be still be here completely lost. I had originally gone to another site for a Warranty Deed & they wanted double the amount of your price & their website had no examples forms. I am so happy with your site & service. The forms were immediately available for download, which was nice. However, I was not impressed by the lack of several features: 1) there was no way to edit set text in the form, such as where it says you should consult an attorney. That is not necessary for recording the deed and I wanted to deleted it, but could not. 2) Also, under the "Notes" section, there is a limited area to write; I tried adding a fuller explanation of something, but the form would not accept or include it when I printed the final document. The form may do the job, but it's not very sophisticated or elegant.
