Confirming a Transfer of Nebraska Real Estate after Death of principle.
Under Nebraska Revised Statutes section 76-214, deeds and other instruments transferring title submitted for recording must be accompanied by a statement containing details about the transfer. Form 521, found on the websites for most counties or at http://www.revenue.nebraska.gov/PAD/forms/521_Real_Estate_Transfer_Statement.pdf meets most of these requirements.
Transfers resulting from a property owner's death, such as those under transfer on death deeds or joint tenancies, may require additional information (76-214(1), (2)). In those situations, including a cover sheet that contains the extra details assists the recording office and reduces the chance of delays in processing.

Product was ok; except in divorce cases there are usually two grantors - your form had only one signature and notary line for a grantor on the Quitclaim deed.

I purchased the Affidavit of Surviving Joint Tenant document and found the whole package of documents to be useful and practical. Successfully recorded!!! While the fillable PDF files are good enough, I personally prefer a Word document as it is easier to modify font or spacing.

This is the first time that I have used this service. An employee at the Clerk and Register office in Arizona suggested that I try Deeds.com to find the form I needed and the county office could not provide. I was skeptical after experiencing other websites. However not only did we get the form we needed for a fraction of the cost vs going to an attorney, the additional resources (guides and samples) made the completion of the Enhanced Life Quitclaim deed quite simple, quick, and painless. We were having difficulty getting my mom to agree to meeting with an attorney or even considering a Lady Bird deed.
