Use this form to create a survivorship joint tenancy in Ohio real property. The Ohio Revised Code Section 5302.17 states that a "deed conveying any interest in real property to two or more persons . . . creates a survivorship tenancy in the grantees, and upon the death of any of the grantees, vests the interest of the decedent in the survivor, survivors, or the survivor's or survivors' separate heirs and assigns."
Once executed and recorded, this deed immediately conveys ownership from the grantor to the grantees. To retain a shared ownership interest in the property, the grantor simply conveys the property to herself and the other intended co-tenants.

It would be helpful to be able to edit verbiage on the form. I was preparing a Deed of Distribution; therefore, there was no consideration paid. I had to type the language into a Word document instead.

Forms used, created quitclaim deed that the county accepted without a second look (turns out they see deeds.com forms regularly via erecording and in person). Website is easy to use. I ordered the form, filled it out and uploaded it for recording. My only critique is that you can't preview the form before ordering and paying for it. I ordered a Deed of Full Reconveyance form only to find out I needed the Substitution of Trustee and Deedn of Reconveyance form instead. So I wasted $22 on the wrong form.
