The Notice of Furnishing is used to certify that labor and/or materials were furnished, or will be furnished upon a specific property identified in the Notice.
In Kentucky, any lien will be dissolved unless the claimant, within six (6) months after he ceases to labor or furnish materials, files, in the office of the county clerk of the county in which the building or improvement is situated, a statement of the amount due him, with all just credits and set-offs known to him, together with a description of the property intended to be covered by the lien sufficiently accurate to identify it, the name of the owner, if known, and whether the materials were furnished or the labor performed by contract with the owner or with a contractor or subcontractor. K.R.S. 376.080(1).

Ultimately, it directed me to the wrong form. Not very helpful. I had to turn to a title company to get my issue addressed.

Disappointed on most recent order. Format did not permit changing the "boilerplate" language to change "grantor" to "grantors". In so restricting, could not use pre-printed form to make a joint party conveyance.

I was very pleased with the service and the product. All the extras were a nice addition to my order. With the example and instructions, I was able to fill out my correction deed correctly. I filed it and it was accepted with zero reservations by my clerk and recorder's office. The expected result (which was to remove a name from the current deed) happened almost immediately. I appreciated the fact that the forms were by Texas County and I knew I had the right form. The form were fairly easy to complete. I had trouble completing the form because the property description was long and kept disappearing and I had to re-type. It would also have helped it I could have saved and not had to start over every time.
