5. Banks and Other Financial Institutions as defined in Section 5-938
6. Operation of Entity or Business as defined in Section 5-939
7. Insurance and Annuities as defined in Section 5-940
8. Estates, Trusts, and Other Beneficial Interests as defined in Section 5-941
Specific Authority is optional, can be granted to any or all as defined by 5-931.
(1). Create, amend, revoke or terminate an inter vivos trust;
(2). Make a gift;
(3). Create or change rights of survivorship;
(4). Create or change a beneficiary designation;
(5). Delegate authority granted under the power of attorney;
(6). Waive the principal's right to be a beneficiary of a joint and survivor annuity, including a survivor benefit under a retirement plan;

A power of attorney must be signed by the principal or in the principal's conscious presence by another individual directed by the principal to sign the principal's name on the power of attorney. A signature on a power of attorney is presumed to be genuine if the principal acknowledges the signature before a notary public or other individual authorized by law to take acknowledgments. A power of attorney under this Part is not valid unless it is acknowledged before a notary public or other individual authorized by law to take acknowledgments.) 5-905(1). Creating my enhanced lifde estate deed was rather easy but I am not happy. After buying the 3 form package for about $29 and crating my legal form... was not happy with your disclaimer than you cannot guarantee the form will meet required legal standards. I was convinced you all knew what you're doing since the forms are supposedly specific to Polk county, Florida.
