Affidavit of Identity and Survivorship for Transfer on Death Deeds -- Minnesota Form 50.2.3
Minnesota's transfer on death deeds are governed by Minn. Stat. 507.071.
To complete the transfer of property rights after the grantor owner's death, the beneficiary must file an affidavit of identity and survivorship in the county where the land is located (subd.20). The affidavit must contain details about the deceased grantor owner, all beneficiaries, surviving or deceased, a legal description of the real estate, recording information from the transfer on death deed, and the address of the individual or entity who should receive future tax statements.
In addition to the affidavit, include an official copy of the grantor owner's death certificate and a clearance certificate from the Minnesota Department of Human Services. All three forms are required, and may be filed as a unified document with references on the affidavit to the death certificate and clearance certificate, but each may also be recorded individually. Note that the transfer is not completed until all three forms are filed for record.
The beneficiary may complete an online request from the DHS website: https://edocs.dhs.state.mn.us/lfserver/Public/DHS-5893-ENG to obtain the clearance certificate.
Individual counties may require additional information -- check with the local agency responsible for maintaining land records with any questions.

If the recorder's office had a form as they like to see, with your name as they like to see, and the property name as they like to see, no one would ever pay a lawyer for this but a little time to look up the exact names and this package you're all set. The service was prompt and attentive to my questions. I would've just appreciated a heads up that I also needed to contact the county directly (and provide contact info) to receive a certified copy of the document (Notice of Commencement) in order to submit the certified copy to the Building Department. This was an extra step that I haven't had to complete before using another eRecording service. Even if this extra step is a result of the county's system. I would still have expected a head's up (since there wasn't any info regarding this on the county's site for eRecording).

Communication is hard. The reps need to be empowered and encouraged to call the customers when necessary. They encourage 300 dpi resolution and under 2 MB PDF file, which is not even possible with our scanner. They made a vague comment about a legal description looking abbreviated but did not explain. They refused to call me. They said the county said "Image is light please darken", but the image looked fine to me. Maybe not their fault, but they refused to help work with the county on that for me. I followed their suggestion though and re-scanned at 300 dpi, but they misunderstood me and did not re-submit it right away. Over 48 hours later, it's still not recorded yet. I hope it will be today.
