I wanna give more stars because the required information is there, but the character spacing is disjointed on the first page, rendering a gap-filled, awkward-looking document. Also, the opening parenthesis for the first field on the first page is on the wrong line and is backwards, which sets the wrong tone especially since it's the first thing you have to fill out.

The Affidavit- Death of Joint Tenant form you provided is not the same form as showed on the Los Angeles County property tax website. It appears that the LA county form requires entering additional info that is not included in your form.

I took a chance and downloaded the Beneficiary Deed form -- would have liked to have been able to see the form before I paid, but I took a chance as everywhere else I looked online wanted me to fill out form online and then pay $30+ for each deed. The deed itself was easy. I did notice that although the website says that the deed would exempt the house from probate, the deed clearly states that it might not. I hope that "might " is the operative word here.
