This is a special durable power of attorney for real property. Durable (stays active, even if the principal becomes disabled or incapacitated). 6. Accept the conveyance or beneficial interest therefor community property with right of survivorship or as joint tenants with right of survivorship with any others, including my said Agent as one of the joint tenants.

Is executed and attested by its acknowledgment by the principal and by an affidavit of the witness before a notary public and evidenced by the notary public's certificate, under official seal, in substantially the following form:
