In Massachusetts, a Notice of Substantial Completion must be filed or recorded with the registry of deeds upon or after the contract has reached substantial completion. M.G.L. Ch. 254, 2(a). "Substantial completion" occurs when the work under the written contract is sufficiently complete so that it can be occupied or utilized for its intended use. Id.
The Notice of Substantial Completion contains the date of the written contact, names of the owner and contractor, and a legal property description sufficient for identification. Id.
The owner also signs the Notice and states that he has served a copy of the Notice upon every person who has filed or recorded a Notice of Contract. Id
A copy of the Notice, indicating the date of filing or recording, must also be mailed by the owner, via certified mail with return receipt requested, to every person who has filed a Notice of Contract. Id. Additionally, the contractor must serve a copy to every person who has entered into a written contract directly with the contractor as well as to every person who has given a written Notice of Identification to the contractor. Id.
The failure of the owner or contractor to give notice of the filing or recording of the Notice of Substantial Completion to the persons entitled shall not prejudice the rights of any third parties who rely upon it in good faith and without actual knowledge of such failure of notice. Id.

Your instructions and sample are geared towards businesses. It would have been helpful to have included some for us individuals as married couples as well. I also recall one or two spelling errors on the form that I could not fix, and the instructions seem to be for a prior form. This particular registry also required a stamped self-addressed envelope for return of documents. Hey, you asked! Overall, pleased.

Delivery of documents was instantaneous once payment is received. Thank you for that. For future clarification to potential users, Deeds.com may want to categorize the type of easement documents that are available. I was needing a 'utility easement' form and received an 'ingress/egress' form. Had I known it was an ingress/egress document, I would not have made the purchase. Outside of this issue, this site is very helpful for the average layperson to hold guardianship over personal interests.

The process for receiving the quitclaim document worked well. I couldn't use the document. If I had been able to view the document before I had to pay for it, I would have known, but that isn't how your process works. However, that's the only snag I found. Otherwise the process for paying and downloading the document worked well. Thank you.

Not totally satisfied as unable to edit as needed on signature page of the deed. I want to be able to date the document and don't want the verbage "signed, sealed and delivered in the presence of" to appear and cannot remove it. The notary will make his or her own statement below as to the date executed.

From Pennsylvania here. Documents are great and easy to fill out however you are lacking a couple of things. You only provide the option for a Grant Deed when you purchase by your county which is Mercer County for me. Why not give the ability to get a Warranty Deed that better protects the Grantee? Also, being from Pennsylvania and in a county that mined Buituminous Coal we are required to include the Coal Severance Notice and Bituminous Mine Subsidence and Land Conservation Act Notice. You can check the box on your Deed form that they are required and attached but you do not provide the verbiage or form for this. You state that you know what each county requires and include everything required but you do not include these two required Notices. This has been a requirement for years and the wording never changes. I had to look for these Notices and hand type this information and include it on another seperate page after the Notary section on the Deed. The Grantor has to sign the Coal Severance Notice and be witnessed by a Notary so I had to add another place for the Notary and will have to pay twice for witnessed signatures when it could have been included in your document. My Deed from 2003 was done that way and then the Notary statement after that so it was only one notarized witness of signature.
